Human Services Case Worker
Summary
The Human Services Case Worker will serve as a trusted connection between Karuk Members and the community, honoring their wellbeing, autonomy, and cultural identity. The Case Worker will take time to understand individual client’s strengths, needs, and goals to work alongside them in developing individualized service plans that are rooted in respect, dignity, and the Karuk Tribe’s commitment to wellness.
Through collaboration with Karuk Tribal programs, local partners, and extended family networks the case worker will ensure that individuals will receive timely and culturally responsive services. This position will work cases ranging from support of local families facing a housing crisis, assisting with the Karuk Tribal Elders seeking to access available healthcare and traditional wellness resources, or advocating for a child’s educational success. The Case Worker should be versed in problem solving through empathy and should empower individuals to achieve stability, balance, and self-sufficiency. This position requires a value in cultural humility and the desire to contribute to the health and resilience of the Karuk People.
Responsibilities
- Develops and maintains an individual’s Person-Centered Support Plan.
- Builds positive relationships with clients, Karuk employees, Karuk service providers, etc.
- Educates individuals, guardians, and families on the informed decision-making process.
- Helps clients connect, apply, and be transported to the necessary community resources.
- Coordinates outreach to communities and participates in events/fairs/gatherings/meetings.
- Coordinates with support team members and providers to ensure accurate and personalized needs are identified, assessed, maintained, etc., as needed.
- Maintains required documentation in eClinicalWorks(eCW) with technical-based skills.
- Participates in intake and referral meetings.
- Facilitates grant applications and renewal processes needed for resources and funding.
- Develops Risk Assessments, Behavior Support Plans, Individual Service Plans, etc.
- Communicates with the Community, State, County, and City collaterals.
- Advocates for all participants served, including those in stressful and crises
- Monitors plan implementation and ongoing support.
- Able to obtain a California Peer-to-Peer or relevant certifications.
- Thoroughly document tasks performed for persons served by Human Services with goal attainment.
- Adheres to Tribe's and Program's confidentiality and personnel policies.
- Be available for local and out-of-the-area travel as required for job-related training and various activities.
- Attends all required meetings, trainings, and functions.
- Be polite and maintain a priority system in accepting other job-related duties as assigned.

Qualifications
- Demonstrated ability to work effectively with Native American youth and family members in culturally diverse environments.
- Displays the ability to establish and maintain harmonious working relationships with other employees and the public.
- Demonstrates the ability to manage time well and work under stressful conditions with an even temperament.
- Demonstrated ability to understand and follow oral and written instructions.
Additional Qualifications
- Demonstrates ability to use a variety of computer programs and, in particular understand, when necessary, operates software such as Windows 10 and MS Office Suite applications.
- Demonstrates strong organizational skills, be able to prioritize duties, and ensure timely completion of tasks.
Requirements
1. Must have an Associate’s Degree in Human Services, Social Work, or a related field.
2. Must have at least one year of experience in case management, human services, probation, elder assistance, or general assistance.
3. Must have a current Cardio Pulmonary Resuscitation (CPR), AED, and First Aid certification, or the ability to become certified within 90 days of hire.
4. Must adhere to the Tribe’s confidentiality policy and Health Insurance Portability and Accountability Act (HIPAA) policies.
5. Must adhere to professional standards, personnel policies, and code of ethics.
6. Must have a positive mental attitude and be able to deal with stressful and unpleasant situations without losing composure.
7. Must provide documentation of immunity to measles, and rubella, or become vaccinated with the recommended vaccines and hepatitis B vaccine. Must test for Tuberculosis (TB) at time of hire and per Nation Centers for Disease Control(CDC) guidelines as required. Must have an annual health exam and a Flu or exemption as required.
8. Must be a self-starter, well organized, and willing to learn new skills.
1. Must possess valid driver's license, good driving record, and be insurable by the Tribe's insurance carrier.
2. Must successfully pass a pre-employment drug screening test.
3. Must adhere to required pre-employment background check, crimcheck and FBI fingerprints. Additionally, (IF applicable): Investigation of character as required by the Indian Child Protection and Family Violence Act(ICPFVP). The minimum standards require an investigation that shall include: a check of fingerprint files of the Federal Bureau of Investigation (FBI) and to appropriate local law enforcement agencies. The applicant must not have been found guilty of, or entered a plea of no contender or guilty plea to any felonious offenses or two or more misdemeanor offense under Federal, State, or Tribal law involving crimes of violence; sexual assault, molestation, exploitation, contact or prostitution; crimes against persons; an offense committed against children.
