Mission Statement

The Karuk Tribe Enrollment Department is committed to serving Tribal people with integrity, accuracy, and respect. Our mission is to safeguard the Tribe’s records, maintain an up-to-date census of our membership, and provide essential enrollment services that honor the sovereignty and identity of the Karuk People.

We support Tribal members and applicants by guiding them through the enrollment process, issuing identification and documentation, and assisting with forms related to cultural, civic, and governmental purposes. Through this work, we ensure that every Tribal member is recognized, counted, and supported as part of the Karuk community, today and for generations to come.

Services Provided

Enrollment Applications

We assist individuals in completing the Tribal enrollment process, ensuring eligibility requirements are met and accurate records are maintained for future generations.

Tribal Member Census

The Enrollment Department maintains a current census of Tribal Members, preserving accurate records that reflect the growth and strength of the Karuk Tribe.

Family Trees

We support Tribal members in researching and documenting family lineage, helping preserve connections across generations and strengthening cultural identity.

Tribal ID Cards

Tribal ID cards are issued as official documentation of Karuk membership and can be used to access Tribal services, programs, and other benefits.

Address or Name Change

The Enrollment Department processes updates to member records, ensuring that legal name or address changes are accurately reflected in Tribal enrollment and census documents.

DMV & Tax Exemptions

We provide forms and assistance for DMV and sales tax exemptions available to members residing on BIA trust land, helping ensure rightful access to these benefits.

Replace an ID Card

Existing tribal members can request a replacement ID card by completing our online form.

Absentee Ballot

Use this form to request an absentee ballot. Please complete all required fields before submitting. Once received, our office will process your request and provide further instructions.

Enrollment Department Forms

Due to the sensitive nature of personal information provided, the following forms must be submitted by mail.

Tribal ID Card Request (New Members)

Newly enrolled members may request an official Tribal ID card, which serves as verification of Karuk membership and provides access to Tribal programs and services.

Tribal Membership/Descendancy Application

This form is used to apply for enrollment or to establish descendancy. It ensures accurate documentation of eligibility and family lineage within the Karuk Tribe.

Census Questionare

The census form helps maintain up-to-date records of Tribal Members. Accurate census information ensures proper planning, representation, and delivery of services.

Name/Address Change Form

Members can use this form to update their legal name or current address. Keeping records accurate helps the Tribe stay connected and ensures proper documentation.

Answers to common questions about the enrollment process.

How do I apply for Tribal enrollment?

To apply for enrollment, you must complete the official Enrollment Application form. Once filled out, the form can be submitted either by mail or in person to the Enrollment Department for processing.

How do I update my information if I move or change my name?

Members can submit an Address or Name Change form with supporting documentation. The Enrollment Department will update your record to keep your information current.

What services does the Enrollment Department provide to Tribal Members?

In addition to processing enrollment applications and maintaining the Tribal census, the department assists with family trees, Tribal ID cards, fishing licenses, and DMV and tax exemption forms.
