FAQ's

For those of you who live too far away to come to Happy Camp to obtain your ID card, please complete the following information and return this form with an appropriate picture. If you plan on coming into the office please call ahead to make sure someone will be in the office. Please return this form with a new, color, head and shoulders photo taken from 2-3 feet away with a light blue or gray background. (You may e-mail your photo, if you prefer. In order to present to the Tribal Council, an agenda request form is required to be completed fully, including an email address and phone number. If agenda request forms are not completed and turned in one week prior to the meeting, the request may be moved to the next available public meeting. Currently all items must be presented to the council in-person at the any of the three Karuk Tribe council chambers in Yreka, Happy Camp, or Orleans.
